Bread of Life Youth Retreat

A weekend By, For, and Of Youth and Young People!

GENERAL INFORMATION

The Bread of Life Retreat will be held Friday, Saturday and Sunday, April 29 through May 1st, 2011, at Guerin Prep High School in River Grove, IL. All high school age youth are invited. REGISTRATION

The Registration Form and Liability Release can be dowloaded here.

All Chaperones and Participants must complete the attached Registration Form and submit the registration fee.

All forms for participants age 17 and younger MUST be:

• Signed by a parent or legal guardian
• Completed and submitted before attending the retreat
• Participants are strongly encouraged to pre-register as the retreat is expected to reach capacity. On-site registrations will be accepted only if space is still available. Any participants who register at the door must have the Registration Form filled out in its entirety, and:
• Youth age 17 and younger must have the registration form signed by a parent or legal guardian
• Youth age 17 and younger cannot attend without a chaperone. Groups must have a ratio of one chaperone for every 8 youth
• On-site registrations are only permitted if space is available

Return completed Registration Form and fee early to save if postmarked by:

• April 1st, 2011 – $35 per person.
• April 2nd, or later – $45 per person.
